| Effects of t-PA most likely occur in the first hour after infusion |

In patients with acute intracranial artery occlusion treated with intravenous t-PA, the majority of recanalisations occur during the first hour after treatment, a study by Dr M Ribo and colleagues shows. Recanalisations occurring after 1 hour were rare but still related to clinical improvement if achieved within 6 hours from the onset of stroke symptoms.
When it occurs, recanalisation happens during the first hour after t-PA treatment initiation," Dr Ribo noted. "If after 1 hour flow improvement is not observed, it probably won't happen." "Thus, it is important to monitor arterial status with transcranial Doppler ultrasound, and if flow improvement is not observed in the first 60 minutes, more aggressive reperfusion strategies such as intra-arterial procedures should be considered."
